It is very common for a good broker to require this....in many states there are laws that require these types of disclosures. It is also a document that can spell out the brokers responsibilities so that he or she can be held accountable for the services they promised to provide. It also demonstrates to the broker that you are making a committment to him/her which in turn you will get more service at a higher level.
